What you get
Small daily habits make the biggest difference. The health log makes them easy to keep.
Enter your blood pressure or blood glucose exactly as your device shows it, and note when you took it: resting, fasting, before or after a meal.
Charts with your average, how often you’re in target and how your readings change through the day. Press and drag to see each reading.
Every reading is checked against standard clinical ranges. If it needs attention, you’re told what to do now, from re-checking to getting emergency help.
A gentle nudge to log your readings every day, so your doctor has the full picture at your next review.
Add your treatment and get dose reminders. Mark each dose taken or missed and see your progress and refill dates.
Set your next review date and book it in the app. A summary of your readings is added to the booking for your doctor.
How it works
Create a plan for each condition you manage: blood pressure, blood sugar or both.
Choose blood pressure or blood sugar, give the plan a name and add your doctor’s instructions and next review date if you have them.
Add a reading whenever you measure. Build your baseline by logging at least once a day for the first week.
See whether you’re on track, spot rising trends early and book a doctor’s review when your readings suggest you should.
Clear guidance
A number on its own doesn’t tell you much. The health log tells you what it means and what to do, and checks for warning signs when a reading is very high or low.
Reference ranges
Blood pressure follows ACC/AHA categories and blood sugar follows ADA targets. They inform you; they are not a diagnosis. Your doctor may set different targets for you.
| Category | Systolic | Diastolic |
|---|---|---|
| Normal | Under 120 | and under 80 |
| Elevated | 120–129 | and under 80 |
| High · stage 1 | 130–139 | or 80–89 |
| High · stage 2 | 140 or higher | or 90 or higher |
| Very high | Over 180 | or over 120 |
| When measured | Target |
|---|---|
| Fasting or before a meal | 80–130 mg/dL |
| After a meal | Under 180 mg/dL |
| Bedtime | 90–150 mg/dL |
| Low | Under 70 mg/dL |

Medication tracker
Add your medication and dose times. Reminders can ring like an alarm, even when your phone is on silent, until you respond.

The health log is part of My Care Place in the Instant Doctor app. It lets you record blood pressure or blood sugar readings, see trends over time, get reminders to log readings and take medication, and tells you when your readings suggest you should see a doctor.
Yes. Logging readings, care plans and medication reminders are free in the Instant Doctor app on Android and iOS. You only pay if you book a consultation with a doctor.
Following ACC/AHA categories, a reading of 130–139 systolic or 80–89 diastolic is high (stage 1), and 140/90 or above is high (stage 2). Over 180/120 is very high: rest for 5 minutes and re-check, and if you have chest pain, breathlessness, weakness or a severe headache, call emergency services.
Following ADA targets, a fasting or before-meal glucose of 80–130 mg/dL and under 180 mg/dL after a meal is in target. Under 70 mg/dL is low: take 15 g of fast-acting sugar, wait 15 minutes and re-check.
Yes. With Family Care you can keep a health log for your parents, children or partner, get their reminders labelled with their name, and be alerted when their readings need attention.
When you book a review from your care plan, a summary of your recent readings is added to the booking so your doctor can go through them with you. You can also share a secure, time-limited care summary link.
No. Guidance in your care plan follows standard clinical ranges and doesn’t replace your doctor’s advice. In an emergency, call your local emergency number.
